Ethics review forms

University Forms

For Staff and Research Students

Staff and research students (registered for an MPhil, MPhil/PhD, PhD or EdD) must submit their application for research ethics review on the UREC Form E2U. The Committee has a multi-disciplinary membership that includes academic researchers across all Faculties and non-research lay members from within and outside the University so please do think about the content of the form. The lay description should be just that, so it can be completely understood by all disciplines and the research methodologies should be well set out so the Committee can easily understand your techniques. For undergraduate and taught postgraduate students research ethics review is undertaken at Faculty level using the E1/E2/E3 forms given below.

  • University Research Ethics Committee form (E2U)
    Applicants should first contact their Department/Faculty Research Ethics Officer (REO) to establish procedures for ethics review in the Faculty. Applicants must go through Faculty procedures and the REO must sign off the application before it is copied and submitted to UREC. Please make sure that you leave enough time for the Research Ethics Officer to read through the application and make any comments if necessary before the documentation is forwarded to UREC.
    Please download the E2U form from this site each time you wish to submit an application to UREC to ensure you have the latest version (current version: February 2012). The E2U form is locked. To enable editing commands, such as spell-check, search and replace etc., unlock the form by clicking the padlock icon in your Word toolbar. To complete the checkboxes, the form must be locked; this may be done by again clicking the padlock icon.
  • Annual monitoring form
    This short form is to report on the conduct and conclusions of the study either one year into the study or at it's completion if less than a year.
  • Final monitoring form
    Similar to the annual monitoring form, to be completed at the end of studies lasting longer than one year.
  • Determining light touch ethics review
    A checklist to determine whether your application merits light touch review by UREC. NB: it is for the Faculty Research Ethics Officer, not the researcher, to decide whether the proposal is appropriate for a light touch review.
  • Application form for light-touch research ethics review
    This is a shorter tick box form for research that may be reviewed by a light-touch method once the above checklist has been completed. It is available to staff carrying out studies that address minimally sensitive topics, entail minimal intrusion or disruption and involve participants who would not be considered vulnerable in the context of the research. Studies may include:
    • research which might be considered audit or service evaluation;
    • research where the subjects are being asked only about their professional (usually job-related) roles;
    • anonymous self-completion questionnaires on a non-sensitive topic;
    • research that replicates a study of yours previously approved by UREC
Light touch review is only available for staff; PhD students must submit their application to the full UREC committee on the E2U form.
  • Determing expedited/'fast track' review
    A checklist to determine whether your application merits an expedited review by UREC. NB: it is for the Faculty Research Ethics Officer, not the researcher, to decide whether the proposal is appropriate for an expedited review.
  • When is ethics review by UREC not required?
    A checklist to determine whether a study should be reviewed at UREC or Faculty level. NB: it is for the Faculty Research Ethics Officer, not the researcher, to decide whether the proposal requires UREC approval.
